One of the benefits of membership with the Arizona Restaurant
Association and the Arizona Hotel & Motel Association is the Worker's Compensation
Insurance plan. This program is especially available to the members of the
ARA and the AzH&MA, and is a non-participating basis. This means there is
no waiting and hoping for dividends to be paid at the end of 18 - to - 24
months. All the savings are given up front through the highest 'deviation (or 'discount')' available in the industry.
In order for us to provide you with a quote, please fill out the
following application that applies to your membership status and fax or mail back
with your Experience Rating (NCCI) Worksheet, which should be dated at the bottom of the page no
longer than 3 months ago. This page verifies your Experience
Modification, and should be sent to you at least quarterly by your current carrier.

report. Upon receiving your application, one of our Program Managers will contact you within 14 working days to review your
quote and answer any questions you may have.
